No video from iLO on XL220n

I have multiple Apollo n2600 chasses with XL220n blades. On several of these systems I am not getting any video output from the iLO when powering on the server. I am using the SUV cable connected to a USB keyboard and VGA monitor. 

When the server is first powered on, the monitor is getting a signal but no output is displayed. If I hit control-alt-delete, the monitor loses the signal for a few seconds and then reacquires it. It appears hit-or-miss on if any given server will display output. Some have never worked, others work intermittently, and a few work consistently. I have tried two different SUV adapters plus a Cisco adapter, all with the same results.

Are you using any KVM in your setup. Do you have issue with only ILO Remote console ?
